Missouri - Obtain a Certificate of Birth Resulting in Stillbirth

By Mail

Mail the completed application form, with a legal size self-addressed stamped envelope and check or money order for the search fee. Do not send cash. Mail to:

Acceptable identification includes

  • Photocopy of drivers license
  • Photocopy of state ID card
  • Photocopy of tribal ID
  • Photocopy of passport or Visa
  • Photocopy of military ID

If you do not have a government issued photo ID, you must provide a photocopy of two of the following:

  • Social Security card
  • Utility bill with current address
  • Bank statement with current address
  • Pay stub (must include your name, Social Security number plus name and address of business)
  • Car registration or title with current address

Only the parent(s) of a stillborn are eligible to obtain a certified Certificate of Birth Resulting in Stillbirth.

Instructions

  • A Certificate of Birth Resulting in a Stillbirth (CBRS) can be issued for any fetal death records on file in the Vital Records Office.
  • Fetal deaths were filed in the Department of Health beginning in 1948.
  • Only the parents listed on the record are able to request a CBRS.
  • A CBRS can be ordered by mail. Mail requests require a completed Certificate of Birth Resulting in a Stillbirth Application, signed in front of a notary OR a clear copy of a photo ID, along with the appropriate fee of $10.

  • Name of Stillborn child
  • Stillborn childs date of birth
  • Hospital and/or City of Delivery
  • Fathers full name,
  • Mothers full maiden name,
  • Mailing address
  • Relationship (Mother/Father)
  • A photocopy of the current government-issued photo id of the person making the application, i.e. valid driver's license.
