1. Go to the designated District of Department of Public Safety that you belong.
2. Submit those necessary documents required for the application
3. Acceptable forms of identification: You can choose either 2 of the Primary documents or 1 Primary and 1 Secondary documents
4. Pay the necessary fees which amounts to $8.00

• Primary documents
  • Drivers license, state issued Identification card or learners permit - must not be expired for more than six months
  • Certified copy certificate of Live birth issued by the Bureau of Vital Statistics
  • INS Documents
  • Court order containing full name and date of birth
  • Military and Military Dependent ID
  • Valid passport : US, Canadian
  • Social security card
  • If applicable, marriage certificate
• Secondary documents
  • Proof of residence with name and address
  • Bank Statement
  • Utility bills
  • Paycheck stub with name and address
  • Lease/ Purchase agreement
  • Bureau of Indian Affairs Card/ Indian Treaty Card. Not Tribal ID Card
  • Medical records from doctor/hospital
  • Military Discharge/Separation papers
  • IRS/State tax forms/W-2
  • Employer ID card
  • Foreign birth certificate. Must be translated by an approved translator
  • Parent/Guardian affidavit. Must appear in person, prove she/he is the parent or guardian and submit a certified/notarized affidavit. (Applies only to minors.)
  • Firearm permit
  • Pilots license
  • School record/transcript. Must be certified.
  • Student ID card. Must contain a photo
  • Vehicle title or Vehicle registration
  • Voter registration card

Duplicate License is usually issued when your drivers license was lost or misplaced. Since you need to have a drivers license every time you operate a vehicle, it is most important that you will apply for a Duplicate License in case of unforeseen events.
