1. To apply for the procedure, click the link above.
2. On the website, simply choose the transaction you are applying for and provide all the required information.
3. Pay the required fee using either American Express, MasterCard, Visa, or an electronic check and it will be mailed to the address on record at the MVA.

In Person

1. You can also obtain a copy of your driving record by filling out an MVA Request for Record (form #DR-057) at any MVA branch office.
2. Submit the completed form along with your present valid identification.
3. Pay the required fee.

By Mail

You may also download the MVA Request for Record (form #DR-057) and submit the form and required fee by mail. Payment must be in the form of a check or money order made payable to the Motor Vehicle Administration (MVA). Checks must have an imprinted name and address. Do not forget to include your driver license number and your home or work phone number on your check.
