1. Report the incident to the nearest police station.
2. Bring the police report when applying to replace for a lost, stolen, or damaged passport.
3. Call the Ministry of Home Affairs and National Security (MHAS) office to personally asked to take an oath or make a solemn declaration.
4. Apply for a new passport.
5. Follow the procedures when applying for an adult passport.
