Apply In-Person

1. The declarant should visit any of the following competent authority, in particular the prefect, the police station, the gendarmerie in the "Public road" section or the head of the guard post near you immediately and express your interest at the receptionist desk.
2. You will then be required to give a full account event {incident} as per your recollection either though a verbal or written declaration, the police officer will then record the accounts of the crime/incident.
3. If the officer in charge is satisfied that the incident report is in order, you will be issued with a signed/stamped a certificate of loss of papers immediately.
   

Please note

• In case you find the lost/stolen document {s} you should notify and surrender the certificate of loss of papers to the police station/legal authority immediately

The loss declaration certificate is valid until you reapply for the lost document{s}

• State a Detailed Recollection of the event (if applicable).

• Locality (i.e. plot number, street, H/No or any nearest structure).
• Identity of the suspect or physical description of the suspect.
• Any weapons used.
• Names of witnesses where possible

Anyone who has lost their official papers/document must make the declaration immediately to the authorized official, in order to avoid fraud and / or the use of these by a third party.
