Apply In-Person

1. Applicants should visit a government certified government or private medical center/hospital near you and express your interest to the reception desk.
2. Please note that some medical center/hospital will require you to schedule an appointment with them first.
3. The receptionist will then open a patient file on your behalf and issue you a payment receipt, upon which you will be required to make payment against the receipt at the cash counter.
4. Wait for your turn at the waiting area for your turn, you will then be ushered to the office of the medical specialist, who has an in-depth knowledge in the field of disability for a physical assessment.
5. The doctor will then fill out the certificate in accordance to his/her finding and sign/stamp the certificate and issued the certificate to you immediately.
