Apply In-Person

1. The applicant {father, mother or guardian of the minor child} accompanied by the child should visit the city/town hall office, department of Civil status near them and express their interest at the customer service desk and,

   • Collect necessary forms and accurately fill them;
   • Attach all the required documents (see required document below); and
   • Submit the documents to be verified and recorded.

2. If the officer is satisfied that the file is in order, he/she will forward the file to the cash counter, you will then be advised to make payment of the application fees, upon which you will be issued with an acknowledgment receipt.
3. Upon making the payment the file is then registered by the registrar of civil status and you will then be issued with the certificate.
   

• Family record book
• Failing that, provide an extract from the register of birth certificates or a birth certificate for each child.
• Valid identity card for the parent/guardian
