1. Submit the filled application form with the necessary documents to the information counter.
2. Obtain a service memo from the counter if you have submitted all the documents.(the date and time at which to collect the registration lost document)
3. Go to the service counter of Transport Authority, on the date specified on the service memo and submit it (after taking a token number) to the service counter
4. Obtain the Registration lost document after filling your details in the book provided by the staff at the service counter.

• If the vehicle is registered under a company, company registration certificate (copy).
• National Identity Card (copy).
• If registered under a government office, seal of that office.
