  1. To obtain a certificate of identity, please make sure you are eligible for applying certificate of identity.
  2. The certificate of identity is provided by the Immigration Department Malaysia, contact and other informations are provided below.
  3. Following application forms should be completed appropriately : Form I - Im 45
  4. Please make sure you have the documents that are listed under the Required Documents section of this page.

Eligibility

Aged 18 and above:

  • Identification card
  • Entry permit

Under 18 years old and have been married:

  • Entry permit
  • Marriage Certificate

Below 18 years (Has Mother or Father):

  • Identification Card (12 years and above)
  • Entry permit
  • Birth certificate
  • Identity Card Mother / Father or entry permit or passport

Below 18 years(Mother / Father died):

  • Identification Card (12 years and above)
  • Entry permit
  • Birth certificate
  • Death Certificate Mother / Father
  • Statutory Declaration Guard

Below 18 years (The mother and father were divorced (mother / father can not be contacted)):

  • Identification Card (12 years and above)
  • Entry permit
  • Birth certificate
  • Identity Card Guard
  • The court order for custody of children

Below 18 years (Adopted child):

  • Identification Card (12 years and above)
  • Entry permit
  • Birth certificate
  • Identity card Legal Guardian
  • Certificate of Adoption




Fees

500.00 fee for five (5) years



Validity
