• Visit the concerned authority personally.
• Fill the application form with the complete information.
• Submit the necessary documents.
• Pay the prescribed fee.

• Applicants identity card or extract of details from the identity card register
• Citizenship Certificate (if any)
• Recent passport-sized photograph of the document owner with a blue background
• Only copies of documents need to be sent for applications made through the post

All copies of supporting documents should be made on A4 paper

  • Spouse of document owner
  • Child of document owner
  • Next of kin (grandfather/grandmother only)

• Application fee - RM 2.00
• Fee for the issuance of document - RM 5.00

Payment is by cash or postal order of RM2.00 and RM5.00 values
