Malaysia - Apply for Emergency Certificate


Procedure

  1. To apply for an Emergency Certificate, go to any Immigration Offices or Malaysian Representative Offices.
  2. Submit the documents required.
  3. Complete the application form.
  4. Pay the fee.





Required Documents

18 years and above

  • Identification Card or Police or Army Identity Card

Below 18 years and those who are married

  • Identification Card or Police or Army Identity Card
  • Marriage Certificate

Below 18 years and if father is alive

  • Identification Card (if any)
  • Birth Certificate
  • Father's Identification Card

Below 18 years and if father is deceased

  • Identification Card (if any)
  • Birth Certificate
  • Father's Death Certificate
  • Mother's Identification Card

Below 18 years and if parents are deceased

  • Identification Card (if any)
  • Birth Certificate
  • Parents' Death Certificate
  • Statutory Declaration of Guardianship

Below 18 years if parents are separated (Father cannot be contacted)

  • Identification Card (if any)
  • Birth Certificate
  • Mother's Identification Card
  • Court's Order
  • Statutory Declaration by mother

Below 18 years if adopted child

  • Identification Card (if any)
  • Identification Card of legal Guardian
  • Certificate of Adoption certifying child's citizenship status

Eligibility

The Malaysian citizen must have:

  • completed the Form F- Im.42-pin 1/97
  • a documentary evidence of Malaysian status
  • two(2) photographs of applicant measuring 3.5 cm x 5.0 cm.




Fees

Emergency Certificate - RM50.00




Validity

An Emergency Certificate is valid for 12 months only.

An Emergency Certificate is a document issued to Malaysian citizens who have lost their passports, persons who were send back to their home country, or for Malaysian citizens who cannot be issued a passport within a specified period.
