• Visit the concerned authority personally.
• Fill the application form with the complete information. Provide clear reasons for requesting the extract.
• Submit the necessary documents.
• Pay the prescribed fee.

• Identity card number of the applicant
• If the applicant's name is recorded in the parent's Citizenship Certificate, Birth Certificate is a must
• If the applicant is the child of the Citizenship Certificate owner and the certificate owner has died, Death Certificate of the parent is a must

All copies of supporting documents should be made on A4 paper

• Fee for the issuance of document - RM 5.00
