Apply In-Person

1. Prepare all the required documents and obtain the application form from the pharmaceutical, Medicine and Poison board office
2. Fill the application correctly and attach all the required documents and pay the application fees
3. Submit your application to the board for verification and approval
4. Upon obtaining your application, the board will send an inspector to inspect the wholesale premises if it meets the required standards
5. The inspector will inspect the premises and write a report based on his finding and send it to the pharmacy committee
6. The committee will review the report and make a recommendation to the board to issue the registration license
7. The board will approve the application and authorise the preparation of the licence
8. Once the license is ready, you will be notified and request to collect it from the board office

• Documents proving qualification of a pharmacist
• Any other documents requested by the board of poison
