Apply In-Person

1. To apply for pre-shoot fitness certificate for animals, please visit the Veterinary doctor, registered with the Veterinary Council of India .The address and contact details of the office can be found in the following Contact Link.
2. Visit the nearest Government registered Veterinary Clinic/ Hospital and get the pre-shoot fitness application form for animals from the relevant department or you can download the application form from the following Application form Link.
3. Please fill the application form with essential information.
4. Attach all the documents that are listed in “Required Documents” section of this page.
5. The doctor will examine the animal to check its fitness and he may order some tests for the same.
6. Please pay the required fees for the application process and other tests at the office as directed by the officials. Please collect the acknowledgement slip for the paid fee. Keep it safe for future reference.
7. After all the test results come back as normal, the Clinic/ Hospital will inform the applicant over a Phone call or mail to collect it through any applicable means.
