1. Visit a health care provider (such as a doctor, physician assistant, nurse, nurse practitioner, or pharmacist) to get any vaccines, medicines, and advice you need before your trip. Going at least 46 weeks before you travel is best, so that any vaccines you need have time to take effect and you have plenty of time to get vaccines that require more than one dose.
2. If you are visiting several countries or countries with a variety of health risks, or if you have a pre-existing health condition, you may need to see a clinician who specializes in travel medicine.
3. If your travel plans include traveling to or from a South American or African country that is infected with yellow fever or is located in areas where yellow fever transmission has occurred (ENDEMIC AREAS), then the easiest and safest thing to do is to get a yellow fever vaccination and a signed certificate. Be aware, the age requirements for vaccination can vary from country to country.
4. After immunization an International Certificate of Vaccination is issued and is valid 10 days after vaccination to meet entry and exit requirements for all countries.

Required for all travelers arriving from a yellow-fever-infected areas.

A vaccination certificate is required for children of all ages. However, Yellow Fever vaccination is not recommended for children under nine months of age. If travel is unavoidable and the child's physician considers vaccination unwise, ask for a certificate on the physician's own stationary stating the child's age being less than one year as a contraindication to vaccination. Although this is in accordance with World Health Organization resolutions, some countries may not honour such a certificate and the infant may be put under surveillance upon arrival.

All travelers should visit either their personal physician or a travel health clinic 4-8 weeks before departure. If you arrive from a country without yellow fever, there is no requirement for a yellow fever vaccination.

Travelers who have a medical reason not to receive the yellow fever vaccine should obtain a medical waiver.

A Yellow Fever vaccination certificate is only required for travellers coming from a country with risk of Yellow Fever transmission. The vaccination requirement is imposed by this country for protection against Yellow Fever since the principal mosquito vector Ades aegypti is present in its territory.

There is no risk of yellow fever in Madagascar. The government of Madagascar requires proof of yellow fever vaccination only if you are arriving from a country with risk of yellow fever. This does not include the US. If you are traveling from a country other than the US, check this list to see if you may be required to get the yellow fever vaccine: Countries with risk of yellow fever virus (YFV) transmission.

A day-biting mosquito transmits yellow fever. In forest areas monkeys may be a reservoir. The risk of infection is small, but the infection serious.

Yellow fever often occurs in small epidemics. The local population is usually vaccinated and with a reservoir in monkeys it is impossible to predict where the non-vaccinated traveller are at risk.

The WHO recommends that all travellers to countries with yellow fever be vaccinated.

Countries with yellow fever can request a valid vaccination certificate from travelers, and countries without yellow fever can request a certificate from travelers entering from a country whith yellow fever.

MEDICAL REASONS NOT TO RECEIVE THE VACCINE:

• Infants under 4 months must not be immunized.
• Persons severely allergic to eggs should not be given the vaccine. Generally, persons able to eat eggs or egg products can safely receive the vaccine.
• It is prudent on theoretical grounds to avoid vaccinating pregnant women, and for non-immunized pregnant women to postpone travel to epidemic areas until after delivery. (Pregnant women who must travel to HIGH RISK areas should be vaccinated.)
• Persons, whose immune systems are suppressed due to HIV infection, or those travelers undergoing treatments for cancers (leukemia, lymphoma, etc.), or receiving corticosteroids, alkylating drugs, antimetabolites, or radiation, in general, should not be vaccinated unless traveling to an area of known yellow fever transmission. (Patients with suppressed immune systems have a theoretical risk of encephalitis due to the yellow fever vaccine virus.)
