A next-of-kin wishing to obtain an original Lithuanian death certificate for their relative must complete all required forms and submit them along with the hospital record of death or proof of death from a doctor if the relative died outside a hospital, and the relative's passport to the Civil Registrar's Office (Marriage Palace).

While registering death, the register office shall make a death record and issue a death certificate.
A death certificate is an official document that records the date and circumstances of a person's death.
Upon registration of the death, the person reporting the death receives a Certificate for Burial or Cremation (green form) and a Certificate of Registration of Death (form BD8), to be used if the deceased received any pensions or benefits.
To order certified photocopies of registration documents or genealogy documents, application must be made in person or by mail as often additional supporting documentation is required.

Only an immediate family member can apply for a genealogy certificate upon proof of death and proof of relationship to the deceased person. This includes individuals such as a spouse, mother, father, brother, sister, grandfather, grandchildren and grandmother.
