Apply In-Person

1. To apply for the non availability of birth certificate, applicant has to approach the Public Works Department.

3. Download the application form from the given link: Application form. Refer form number 10.
4. And also get a verified statement from the office authorities stating the reason for non-availability, on their letterhead or on the application form.
5. Make sure that you have all the proper documents mentioned in the “Required documents” section below.
6. Submit the form to the municipal office along with the required documents.
7. Refer the office location mentioned in the “Office location & contacts” section below.
8. Police verification will be done if you enclose any other supporting documents except school certificate.
9. After successful verification, concerned authorities in the office will issue the certificate.

2. Please go the respective office.
3. Application can be obtained from the authorities. Please pay for application form as authorities quote.
4. Submit the complete application along with required documents to the operator in the centre.
5. Operator will check the details and start to process online using the computer.
6. Applicant will give the transaction ID for the application made. Please keep it safe for future reference.
7. This application will be processed by the respective department as per how it will be processed in person.
8. Applicant will get notification about the status on his registered mobile number that was provided while applying.
9. Applicant has to go and meet the respective authorities upon notification with documents as instructed to get the certificate as per notified date.

• Applicant has to get a statement from the respective authority on the letterhead or on the prescribed application form, stating the reason of unavailability of birth certificate.
• Along with the statement include the secondary evidence mentioned in “Required documents” section.

• Birth certificate is the mandatory document at the time of school / college admissions.
• Without birth certificate, getting PAN card, Aadhaar card, Voter ID, Driving license, Passport or visa is impossible.
• NABC will fulfil the requirement of birth certificate.
• NABC certifies the registration of the unregistered birth.
