Apply In-Person:

1. To Cancel the Drug license, Visit the Drugs Controller & licensing Authority, Thiruvananthpuram is the nodal office for Lakshadweep drug license related matters.
2. Applicant shall contact district health service office and find out any updated information for cancellation. Please get the contact information through any one of the above mentioned contacts or meet your local administrative department to avail the information.
3. Collect the license cancellation form from the authorities or write it on an A4 sheet.
4. Next fill the form and submit the application form along with original Drug license to the designated authority at District health service or Drugs Control Office of your area.
5. Required documents can be referred from the required documents section below.
6. The submitted form will be checked by authorities for details and norms.
7. This application will be processed further.
8. Drug inspector or the respective authority will verify everything and once all the above process are over controller of drugs or the appropriate authority will Cancel your drug license.
9. Applicant will receive information about the Cancellation of license.
