1. Prepare the following documents:
   1. Copy of the civil ID of the sponsor
   2. One (1) personal photo (4 6 cm)
   3. Copy of the work permit or the visa
2. The applicant, sponsor, or official representative of the sponsor must apply for the procedure in person. Go to any of the five Health Insurance Centers depending on your place of residence, and submit the required documents to register your data in the health insurance automated system.
3. Once your data is entered in the health insurance automated system, the health insurance notification will be stamped and signed by the authorized employee in the Health Insurance Department. Then, you will receive the notification form which you will need to submit to the Ministry of Interior in order to have the residency stamp printed.

• The applicant, sponsor, or official representative of the sponsor must apply for the procedure in person.
• Data of the applicant residents working in the private sector, the applicants joining a provider, and some of the applicant servants must be registered in the health insurance automated system after cash payment.
• Data of the applicant employees in the governmental entities and the Zakat House must be registered in the health insurance automated system with an official letter from the entity concerned
• Data of the applicant servants must be registered in the health insurance automated system after paying the fees in stamps
