1. To Apply for Medical Certificate, the applicant has to approach the nearest clinic or government hospitals or National Health Mission – link Kerala State Medical Council – link
2. The applicant has to download the form from the following link or from the hospital/clinic - link
3. The applicant has to enter the details in the form.
4. Submit the application form to the doctor.
5. If prescribed, the applicant has to undergo for some test.
6. Once the test result gets done, the applicant gets the certificate from the doctor.
7. The processing time for this procedure is within 2 days, if any test is done.
8. The fee for this procedure has to be paid as per the authority’s request.

NOTE: Some corporates have their own format for this certificate. In such case, one has to get the format and get certified with the medical practitioner

• Medical certificate is a requirement to those who wants to obtain a driver's license and other official documents
• As a policy MNC’s will ask the candidates to submit a medical certificate
• Few Govt. service requires medical certificate to ascertain the fitness

• Minimum of Rs.200 will be charged ( This fees changes from Doctor to Doctor)
• Medical certificate which is meant for overseas job requires certain level of examination as advised by the recruiter from overseas and fees for such certificate will be decided by them
