This application is done in person however you can initiate the process by calling the police through their designated hotline landline numbers of 999 or pressing the emergency button in your cellphone
Apply In-Person
- Get particulars of the crime incident including: time, nature, whether there is loss of life or property involved, witnesses, and clues that could help the police identify the criminals.
- Proceed to call the police through their emergency numbers so that they may report at the scene of the crime.
- If the crime does not require a very urgent attention, you may visit the nearest police station to make the report.
- The police will try their best to respond to a crime in the shortest time possible. Normally you will still be required to go to the police station to record a statement.
- If you go to the station the officer in charge will record your statement about the crime in the OB book (Occurrence book) normally at the reception desk.
- Give your statement; giving sufficient details on the crime (the police officer at the desk will guide you through by asking you the necessary questions).
- If your case is very urgent, you may want to see the Officer in Charge of Station (OCS), if he/she is in a position to arrange an immediate visit to the crime scene.
- A crime in which a personal injury has been sustained requires you to obtain a P3 report. The P3 form is provided free of charge at any police station and should be filled by the police who receive the crime report and a medical examiner.
- If in any case there is loss of property then police will issue an abstract with particulars of the lost property.
- If it’s a road accident an accident abstract will be issued.
- After making the initial report and recording a statement, you might be directed to a different section to report crimes of specific nature. For instance, gender violence, child molestation, among others.
- You will also be issued an OB number for reference
- At the end, the perpetrator is summoned or a warrant of arrest issued.

Need for the Document
- Reporting a crime is very important for the following reasons:
- Police do not automatically investigate a crime. They can only investigate if the crime has been reported and a statement made.
- Reporting the crime is important for accurate crime stats. This gives the public more leverage to request extra police officers, vehicles etc.
- It gives the police a time frame & points out “hot spots”
- Polcie might be able to lift fingerprints that can possibly link them to existing or future cases.
- Criminals always leave something behind.
- Be as factual as possible when reporting a crime .
Other uses of the Document/Certificate
- The report is used as a basis in criminal proceedings and charges in a court of law.
