1. The application for a workplace registration certificate shall be made to the Directorate of Occupational Safety and Health Services Office(Workplace Registration Office).Ministry of Labour Office Location & Contact Details
2. Applicants are required to make payment for the registration fee of Ksh 5000.00. The fees are payable to Kenya Commercial Bank, Account number: 1130057321, Account name Occupational Safety and Health Fund, Industrial Area Branch and obtain a Bank deposit receipt.
3. The application for the registration certificate is done on the prescribed application form: obtainable at the Directorate of Occupational Safety and Health Services Office or directly download it using this link: Application Form for Registration of a Workplace
4. Upon obtaining or downloading the applicable Application Form, fill it out and submit to the Authority the completed registration form and required supporting documents as stipulated on the “Required Documents” section of this page.
5. On receipt of the application file, the receiving officer shall check for completeness of the form to ensure it meets the registration requirements and registers your application as received by issuing the applicant a DOSH(Directorate of Occupational Safety and Health) official receipt.
6. The receiving officer will forward the application and supporting documents to the next level officer for further scrutiny and confirmation that the application complies with all the requirements.
7. After a successful documents evaluation process which is usually within 7-15 days, the director shall approve the registration and issue the workplace registration certificate, where he is satisfied that the application meets the requirements.
8. Applicants should visit the Director of Occupational Health and Safety Services (Workplace Registration Office) to collect the workplace registration certificate against their identification document.
9. Where the Director of Occupational Health and Safety Services refuses to issue a certificate of registration, he must, if so required by the applicant, state in writing the grounds of such refusal, and the applicant may appeal the decision.

Note: A person who is aggrieved by a decision of the Director made under this section may, within fourteen days from the date of the decision, appeal to an appeal committee appointed by the Minister of Labour.

• Any person that wishes to convert/use a premises into a workplace is eligible to make the application.

• The registration fee of Ksh 5000.00.

• The validity of the Workplace Registration Certificate is indefinite unless revoked, suspended, or cancelled by the Authority.

• It is an offense for any person to occupy or use any premises as a workplace without first having been issued a certificate of registration.
• The occupier of a workplace who fails to comply with the registration of a workplace commits an offense and shall on conviction be liable to a fine not exceeding one hundred thousand shillings or imprisonment for a term not exceeding three months or to both.
