Obtaining an ID Card:

1. Complete and submit the application form at the Circuit Court Clerk's Office in your county of residence.
2. Submit the required documents.
3. Pay the applicable fee.

Change of address:

Name or address changes must be reported within 10 days. To apply for changes in your ID card information, you must show documentation in the form of a marriage license, divorce decree or name change order. All names on the documentation must match.

• Applicants must show an original or certified birth certificate, Social Security card, proof of address and marriage license if applicable.
• Applicants under the age of 18 will need a parent or guardian to sign the application. Guardianship papers are required.

To apply for an ID card, you must be:

• A legal resident of Kentucky
• At least 15 years old

Note: The State of Kentucky issues child ID cards to those who are 2 years old to younger than 15 years old.

Identification card cost:

• For applicants age 15 or older: $12
• For applicants under the age of 14: $4

Identification cards are valid for four years
