Apply In Person

1. To Apply for Pre-Shoot Fitness Certificate for Animals, the applicant has to approach the Veterinary doctor, registered with the Veterinary Council of India – link
2. Visit the nearest Government registered Veterinary Clinic/ Hospital and get the pre-shoot fitness application form for animals from the relevant department or the form can be download from the following link – link
3. The applicant has to enter all the details in the application form.
4. Attach all the necessary documents which is mentioned in the “Required Documents” section of this page.
5. Submit all the documents to the doctor.
6. The doctor will examine the animal to check its fitness and he/she may order some fitness test.
7. Please pay the required fees for the application process and other tests at the office as directed by the officials. Please collect the acknowledgement slip for the paid fee. Keep it safe for future reference.
8. Once the report comes, the clinic/Hospital will intimate the applicant through phone to collect the report.
9. After completion of all the fitness test, the doctor will intimate the processing time.

• Proof of identity for Applicant and Owner: (Voter ID Card/Pan Card/Aadhaar Card/any ID Proof)
