1. Submit a written request indicating the name of the person and the national number or the serial number of the certificate holder required from only an adult family member.
2. The request is submitted to any office of the department in case the certificate information is not entered by the computer then the transaction is forwarded to the office where the incident required is registered.
3. Present the identity card or Family Book containing the incident to which the certificate is required / passport of the applicant for non-Jordanians.
4. Pay legal fees.
5. Print death certificate, stamped and signed.
6. Deliver the certificate to the citizen.
