To replace an adult passport:
- To apply for replacement of a Lost, Stolen, or Damaged Passport, you should visit the Passport, Immigration and Citizenship Agency (PICA) head office or regional offices if you are in Jamaica. You can view PICA’s location map and contact details via this link: Link if you are staying abroad, you can submit your application through the Jamaican consulate in the country you are residing in.
- Go to the PICA’s offices near you to obtain the application form or download it electronically via PICA’s website: Link (Passport Application Form) or download it directly via this link:Link
- Read the instructions provided in the form and fill it correctly.
- Visit the nearest police station to you and report the circumstance in which the passport was lost, stolen, or damaged, and obtain the police report
- Prepare the rest of the required documents and attach them to the application form including the police report
- Then visit PICA’s office and pay the application fee according to the service you wish to apply for i.e., normal service or expedited service at the cashier, and obtain the payment receipt
- Upon payment submit the complete application to the passport service unit for verification
- The PICA official at the service unit will validate your application if it is complete as required will enter your data into their system to cancel the initial passport. Note that you should submit a damaged passport as part of the required documents
- Upon entry of data of your application, you will be issued an acknowledgment receipt containing details of your application
- Your application will be processed and a new passport will be issued to you
To replace a lost/stolen or damaged minor (under 18yrs) passport:
- Minors who are under 18 years of age should be accompanied by their parents or a legal guardian when applying for replacement of a lost, stolen, or damaged passport, and a complete application should be submitted to PICA or through the embassy if you are staying abroad
- Go to the PICA’s offices near you to obtain the application form or download it electronically via PICA’s website:Link (Passport Application Form (Regular)) or download it directly via this link:Link note that as the parent or legal guardian applying,you are required to complete section C of the application form
- Visit the nearest police station and report the circumstance in which the passport was lost, stolen, or damaged, and obtain the police report
- Prepare the rest of the required documents and attach them to the application form including the police report
- Then visit PICA’s office and pay the application fee according to the service you wish to apply for i.e., normal service or expedited service at the cashier, and obtain the payment receipt
- Upon payment submit the complete application to the passport service unit for verification

- Note: If you the parent or legal guardian is out of Jamaica, then write a letter of consent giving the guardian or representative permission to apply for and collect the passport on your behalf. The consent letter should be notarized by an authorized person from the country you are in and send along with a copy of your passport or acceptable ID to the adult who will be doing the application through PICA’s office. The applicant is also required to present their ID.
Application from abroad
- Applications from abroad are to be submitted to the nearest Jamaican Consulate/Embassy but you must first obtain a police report stating the circumstance in which the passport was lost, stolen, or damaged
- Note that different embassies have different policies so you should contact them in advance to request if you need to book an appointment for submitting your application

- The official police report
- Damaged passport
- Birth Certificate or Adoption Certificate or Jamaican Citizenship Certificate
- Two identical Photographs (Note: one photograph must be certified by the same certifying official who certified your application)
- A valid Government-issued Photo Identification (Example: Driver’s License, Voter Identification).
- Proof of name change if the name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)
To replace a lost/stolen or damaged minor (under 18yrs) passport:

Adult Replacement
- In Kingston (7 workings days), Montego Bay (14 working days) Portmore and May Pen Tax Offices (11 and 14 days respectively) Overseas applications (20 working days upon receipt in Kingston), the application fee is:JA$ 11,500 for Local application fee and $160 or £100 for overseas application
- Rush 3 Working Days: JA$14,500
- Regular for Adult-Rush Next Day JA$16,500
- Regular for Adult-Rush Same Day: JA$21,500
Minors Replacement
- In Kingston (7 workings days) Montego Bay (14 working days) Portmore & May Pen Tax Offices (11 and 14 days respectively) Overseas applications (20 working days upon receipt in Kingston),the application fee is:JA$7000 for the Local application fee and $100 or £50 for overseas application
- Rush 3 Working Days: JA$ 9000

Validity
- The Jamaican passport is valid for ten years for an adult and five years for children 17 years and under.

Instructions
Collecting a passport
Passports are issued in the same PICA office where you submitted your application.
- To collect your passport, you will need to present the official receipt.
- For passports issued to minors:
- If you were the parent/legal guardian that applied, you will need to produce the official receipt and a valid photo identification
- If you are collecting for a minor passport for which you did not apply, you will need to present the official receipt, written consent from the parent/legal guardian who applied, a copy of valid photo identification for the parent/legal guardian who applied, and your valid photo identification.
- If you are collecting on behalf of another then you will need to present the official receipt, a letter from the applicant permitting you to collect his/her passport, and valid photo identification.
- Should a lost passport be recovered, it should be returned to the passport office immediately
