Apply In-Person

1. To apply for Maternity Benefit under the National Insurance Scheme (NIS), you have to submit your application to the NIS office under the Ministry of Labour and Social Security (MLSS). please view the NIS contact details in the “Office Locations & Contacts” section below
2. Obtain the application form from the NIS office in person or download the form from the MLSS website through this link:Link (choose “Maternity Benefit” and download the form)
3. Read the instructions provided and complete the form in the presence of a witness. If the applicant is unable to read or write, the form can be completed by a witness on behalf of the applicant. The witness should also sign PART II (4) of the application form
4. Then gather and prepare the required supporting documents and attach them to the application form
5. Once your application form and all supporting information and documents have been completed, signed, stamped, and certified where required, go to the social security office in your parish and submit the application to the NIS official at the service desk
6. The NIS officer will verify your application and confirm if the application is completed as required then authenticate the date of receipt of the application as required in the official use section
7. The official will submit the application to the necessary authorities for assessment and approval
8. The application will be reviewed and if it meets all the required standards for maternity benefits, it will be approved
9. After approval, you will receive the maternity allowance through the most convenient way from the social security office, NIS division
