Apply In-Person

1. To declare your age to the National Insurance Scheme (NIS) Jamaica, you should submit theage declaration form to the NIS office under the Ministry of Labour and Social Security (MLSS). Please view the NIS contact details in the “Office Locations & Contacts” section below
2. Obtain the application form for age declaration from the NIS office in person or download the form from the MLSS website through this link Link (choose “Age Declaration” and download the form)
3. Complete the application form correctly in the presence of a Justice of the Peace (JP) official and have the JP authenticate the JP’s declaration. Note that the form should be completed by you and a second declarant who knows you well
4. Then prepare the required supporting documents and attach them to the application form
5. Once your application form and all supporting information and documents have been completed, signed, stamped, and certified where required, visit the NIS office and submit the complete application to the NIS official at the service desk
6. The NIS will verify your application and confirm if the application meets the required standards for the age declaration
7. Upon verification, your declared age will be duly registered with the NIS

• Valid Identification of the Applicant (Passport, Driver’s Licence, Electoral Identification)
• Deed Poll if applicable
