1. A request for the application should be sent along with the required documents to the Medical Professions Licensing Department.
2. After receipt of the application, the applicant's eligibility to sit the examination is assessed by the Medical Professions Licensing Department. A notification will be sent to eligible applicants summonsing them to an examination, to the address held in Ministry records.
3. The applicant will be asked to attend an examination and the applicant should clear the examination.
4. After clearing the examinations the applicant will go through a criminal record check which the applicant should clear after which the license will be issued to the candiate.

• 2 passport photographs.
• Photocopy of your identification card, including the attachment showing your address.
• Final diploma in occupational therapy, or alternatively certification from the university that you have completed your studies and have fulfilled all obligations to the university and are eligible for a degree in this profession, which will be awarded at a certain time.
• Official confirmation of the studies start and completion dates.

• Successful completion of government examination
• Age at least 18.
• Citizen or resident of Israel.
• Has no incapacitating medical condition.
