People choose to change their name for a variety of reasons. You can legally change your first name, middle name or surname, or any combination of those.

You must appear in person and present your identity card, passport and current picture and those of your spouse when you submit the notification.
If the change is for all members of the family or for minor children, the presence of both spouses (the parents) is required.
An Israeli citizen who wishes to change his first or last name must come in person to the mission, where he will fill out the notification form on the change of name in two copies (click here to download the form Notification of Change of Name). The mission will send the form to the Ministry of the Interior in Israel. When the change of name is approved, the passport will be updated accordingly. When a citizen who has changed his name returns to Israel, he must go to the Ministry of the Interior to have the change made in his identity card.
