• Fill out the application form
• Send the completed application form, other required documents and your cheque payment to the Isle of Man Welcome Centre.

Lost or stolen cards

In case you lose your 18+ card please complete the Declaration of Lost or Stolen Proof of Age Card form and take it to the Welcome Centre as soon as possible.

You need to pay 10 replacement fee that will cover production and processing costs.

Applying in person:

• Completed application form
• Applicant's original (or certified copy) birth/adoption certificate or current photo driving licence showing your date of birth and a recent passport quality photograph of the applicant authorised by his/her Counter signatory OR
• Applicant's current passport or current EU identity card and a recent passport quality photograph of the applicant

Applications submitted by post:

• Completed application form
• Applicant's original (or certified copy) birth/adoption certificate and a recent passport quality photograph of the applicant authorised by his/her Counter signatory

To qualify as a counter signatory you must be:

• Resident on the Isle of Man
• Have no familial connection with the applicant (including guardian, foster parent or carer)
• Have known the applicant personally for at least 2 years
• Be over the age of 25
• Be a professionally qualified person: accountant, advocate, architect, bank or building society officer, civil servant (permanent), chiropodist, college lecturer, coroner, dentist, dispensing chemist/pharmacist, doctor, driving instructor, fire service officer, Justice of the Peace, legal secretary (member or fellow of the Institute of Legal Secretaries), local authority officer or representative, Manx Electricity Authority management, Member of Tynwald, midwife, minister of a recognised religion, nurse, person with honours (eg OBE, MBE, etc), optician, police officer, Post Office officer, Salvation Army officer, schoolteacher, surveyor or valuer, veterinary surgeon, youth worker.

Counter signatories should sign, date and write the following endorsement on the back of the applicant's photo: I certify that this is a true likeness of (applicant's name).

• For application forms submitted in person: Payments may be in cash, credit/debit card or cheque (payable to Isle of Man Government)
• For application forms submitted by post: Cheque payable to Isle of Man Government

Proof of Age cards are valid for 5 years.

You may purchase a Proof of Age Card in person or by post from the Welcome Centre in Douglas Sea Terminal which is open from 9:00am to 5:00pm, Monday to Saturday.

A list which displays the kind of information which is required to complete the procedure.

Isle of Man 18+ Proof of Age card enables its holders to prove they are old enough to purchase cigarettes and tobacco products, alcohol, fireworks, films, solvents, videos and petrol without carrying around other documents such as a passport, which would be costly to replace if lost.
