1. Go to the police and file an FIR informing them the loss of the voter card.
2. Keep the copy of the FIR handy.
3. Obtain the 002 form (for issuance of duplicate voter ID card) from your local election commission office or from your state election commissions website.
4. Fill out the form and complete it.
5. Attach the copy of the FIR.
6. Attach a document that certifies your identification proof. This could be a copy of your passport, driving license, pan card, or ration card.
7. Attach a document that certifies your place of residence or address. This could be a copy of your gas bill, electricity bill, telephone bill, and other utility bills.
8. Submit the form along with the documents to your local Chief Election Commission office.
9. A receipt will be given to you upon receiving the documents.
10. Check the status of your duplicate voter ID from time to time to know when you would receive the voter ID card.
