India - Apply for Journalist Health Card


Procedure

Apply In-Person

  1. Application form to Apply for a journalist health card is available in the enrolment centre.
  2. The applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
  3. The applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
  4. There is no charge for a Journalist health card.
  5. Concerned authorities will look into the application form and submitted documents before providing an approval.
  6. Approval will be provided within 2-3 weeks.
  7. If the approval has not been given within 3 weeks, applicant can visit the office to know the status of their application.

Eligibility

  • The applicant should have at least 15 yrs experience in the field.

  • AAdhar card and the photos of journalists shall be given in jpeg format not more than 200 kb in pendrive in the I&PR Dept., for uploading.

  • Journalist’s health card helps journalists and their family member’s medical treatments funded by CM Relief Fund.

Other uses of the Document/Certificate

  • Only such journalists and their family member whose names an photos appear in the cards will be eligible to obtain medical treatment under the scheme. The choice of the hospitals would be with the journalist concerned.
