1. Obtain the proof of death from the Hospital or from a doctor.
2. One have to go to the local municipalitys registry office (Anyaknyvi Hivatal) and request the certificate.

There is a section called finding an office("Hivatalkeres?") on the bottom of the site. One have to select Anyaknyvvezet? in the drop down list, then next to the Telepls neve (place of birth, the city or village) then the Keress button will show the results. It will show the exect place on a map with contact information as well.

It can be requested by a relative or by someone who has interest in judicial proceeding to obtain it.

It is free to obtain it, but if it is lost, 2000 Forints have to be paid to request a new one.

It is Valid Forever.

It is issued by the local municipalitys registry office (Anyaknyvi Hivatal), or at a Hungarian embassy. It is also possible to give someone else authorization for the request.

2. An affidavit specifying the date and time of death
3. Personal identification data of the deceased
4. The official doctor reports
   

Person requesting the death certificate is required to provide the evidence of relationship with the deceased and complete address with nationality.

A Death Certificate is a document issued by the Government to the nearest relatives of the deceased, stating the date, fact and cause of death. A death have to be reported on the next workday as latest.
