Apply In-Person

1. To obtain a death certificate for the animal, the applicant must approach the Veterinary Hospital of their respective area. The address and contact details of all the Veterinary Hospitals of Haryana can be found in this Link
2. Applicant must register the death of the animal by filling the prescribed form.
3. Collect the applicable application form from the office.
4. Fill the application form with the required information.
5. Attach the required documents as per details given in the “Required Documents” section of this page.
6. Submit the application form at the Veterinary Hospital office and make the required payment.
7. Death Certificate is then issued after proper verification.

• Certificate from Employer of reputed companies on letterhead
• Spouse’s passport copy
• Parent’s passport copy, in case of minors(First and last page)

• Must be a licensed owner of an Animal with Pin number.

• Payment will have to be made as per the requirement of the Office.
• Additional fees may be charged for the process.

• The death certificate of Animal is valid forever.
