1. License to operate a pharmacy is issued by the Directorate of Pharmacy Medicines and Traditional Medicine which is also part of Ministry of Public Health and Population.
2. Make sure you all the listed documents that are under section Office Locations & Contacts of this page.
3. A letter from Chief Pharmacist seeking an operating license accompanied by documents that are listed under the Required Documents section of this page should be submitted to the Ministry of Public Health and Population.
4. After verification and validation of the documents and the pharmacy desired location and space a license will be provided.

Following standards should also be met:

• Saturation control in pharmacy
• Adequate space (area 25 square meters minimum)
• Distance of 300 meters between pharmacies
• Sign (+ pharmacy name
• Clean and sufficient space
• Security doors and windows
• Glass shelves painted in oil
• Walls painted in oil
• Hygiene - Refrigeration
• Office Pharmacist expediter (2 Registers) Cardex
• Graduated cylinder - Funnel - Distilled water
• Seal - tiquette- paper with header;
• Pharmacology Book - Vidal or PDR - Toxicology

• Photocopy (2) of Diploma responsible pharmacist (original to be presented)
• Photocopy (2) of the license (original to be presented)
• Photocopy of current identity card,
• Two (4) passport photos,
• Photocopy of current identity card
• Owner identity card (original and copy) and a photo ID
• Criminal record of the owner and head pharmacist
• CV and two (2) letters of reference for the responsible pharmacist (the letters from relatives or allies are not accepted)

This procedure provides you information about how to open a Pharmacy in Haiti. The opening of all pharmaceutical establishment is subject to an operating license issued by the Directorate of Pharmacy Medicines and Traditional Medicine MSPP. This authorization of operation is issued in accordance with standards established by the MSPP and is valid for one fiscal year. These schools are evaluated through inspections periodically.
