Apply In-Person:

1. To cancel drug license you can approach the health and family welfare directorate.

3. Collect the license cancellation form from the authorities or write it on an A4 sheet.
4. Next fill the form and submit the application form along with original Drug license to the health and family welfare directorate.
5. Required documents can be referred from the required documents section below.
6. This application will be processed further.
7. Drug inspector or the respective authority will verify everything and once all the above process are over controller of drugs or the appropriate authority will Cancel your drug license.
8. Applicant will receive information about the Cancellation of license.

• A drug license is required to sell/distribute/store medicine. It can be obtained from appropriate government department. If you are not going to continue your business or else you are going to do any other business you can cancel the license.
