1. Obtain all necessary documents.
2. Proceed to the RENAP Office located at the National Hospital and submit all the required documents to register.

Registration of birth by the father

• Report of Birth in original and photocopy
• Neighbourhood DPI or the Certificate in original and photocopy (Father)

For single mothers

• Report of Birth in original and photocopy
• Neighbourhood DPI or the Certificate in original and photocopy (Mother)

Mother is younger than 14 years old

• Report of Birth in original and photocopy
• Recent birth certificate of the Mother
• Neighbourhood Certificate or DPI of the maternal grandparents in original and photocopy
• DPI or Neighbourhood Certificate of the respondent in original and photocopy

Mother is between 14-17 years old

• Recent birth certificate original and photocopy
• Report of Birth in original and photocopy

Mother is a foreigner

• Valid passport of the mother in original and photocopy
• If the passport is not valid, hand over a General Letter
• Report of Birth

Registration of Birth may be done at RENAP sites located in the National Hospitals.
