You can replace your lost social security card by:

Apply in person

• Preparing all the required documents as provided in the link in required documents section.
• Downloading application form ss-5 in Documents to use section below and fill according to the instructions provided in the application form.
• You can visit that social security office to submit your application in person or you can attach all the required documents to a signed filled application form and mail it to the relevant social security office.
• Your application will be processed and issued to through the mail address you provided.

Application by mail

You can complete the application by mailing your signed application form and documents to the relevant office of social security, Remember that all mailed documents will be returned to you by mail though the mail address you have provided.
