Guam - Renew Sanitary Permit


Procedure

  1. To renew your Sanitary Permit, applicants will need to submit a completed renewal formand pay the application fee.
  2. Applications for renewal of Sanitary Permits can be obtained from the Department of Environmental Health office or you may download a copy from DPHSS website. Note: The link is provided under 'Documents to Use' section.
  3. Applicants will need to submit the completed form together with a copy of the business license and a copy of the current sanitary permit.
  4. Completed applications for renewals that will be paid by check may be dropped off at the DEH office from 8:00 a.m. to 5:00 p.m., Monday - Friday. The applicant will be notified by phone when the permit is available for pick-up.
  5. Applicants who will be paying using cash or credit card will be required to make the payment to the cashier, and wait for the Sanitary Permit AFTER submitting the renewal application and the copy of the business license to the DEH office.

Fees

Sanitary Permit Fee:

  • If the establishment has 1 to 9 employees: $220.00
  • If the establishment has 10 or more employees: The fee is $290.00 and Five Dollars ($5.00) for each additional employee over 10.
