1. You will need to complete the Emergency Passport Application Form (Form D). A link is provided under "Documents to Use" section which you can use to download the form.
2. Attach a photo to your application.
3. Submit your completed application together with other required documents to the Passport Office.

If you are outside Grenada: You may use the link provided under "Office Locations & Contacts" to get in touch with the nearest Embassy of Grenada in the country you are currently staying. Submit your application together with all required documents to the consular service section.
