Step 1 pick the replace form from Ghana immigration service offices in Accra,

Step 2 Fill the form following the instruction as indicated on the form.

Step 3 Report the loss of the work permit to the nearest police within the next 24 hours and obtain reference number from the police.

Step 4 Visit the passports administration office and with all the supporting document and report the loss of your work permit.

Step 5' Visit the concerned authority at Ghana immigration service.

Step 6 Submit the necessary documents.

Step 7 Pay the prescribed fees

i) National identification CardValid passport

iii) Two 2 recent colour photos (4 x 6 cm in size)

iv) Police report or Statement/Affidavit from the police stating how the permit got lost or damaged.
