Apply In-Person:

1. To Extract from Birth Record in Ghana, the applicant has to approach the ADA East District Assembly office or Birth and Death Registry Headquarters office.

3. Applicants can get their application form from the relevant department.
4. Applicant has to enter all the information in the application form.
5. Applicant must make sure to carry all the necessary documents that are mentioned under “Required Documents” section of this page. (Both photocopies and originals)
6. Fees for this procedure has to be paid as per as authority request.
7. Applicant should attach photocopies of documents (whichever is necessary) along with the applicant from while submitting it at the office.
8. Once the applicant submits all the documents, it is possible to obtain a Birth record from the birth register.
9. This process will take minimum of 14 working days.

• A birth extract is a certified true copy of the birth certificate. It replaces a lost or damaged birth certificate.
