Apply In-Person:
- Applicant visit to the Register general department office in Ghana and the applicant can get their application form for change your name(deed poll) contact link
- Applicant must obtain the application form from relevant department.
- Applicant has to enter all information in the application form.
- Applicant must make sure to carry all necessary documents that are mentioned under “Required Documents” section of this page. (Both photocopies and originals)
- Please refer to the fees details in the “Fees” section of this page.
- Applicants are then to swear to an Affidavit of Change of Name (Deed Pool) before the Commission of Oaths, in the Ghanaian court. The affidavit should say that you have changed from your old name and wish to be known by your new name, and you show your affidavit anywhere you want to present any documentation in your old name. The affidavit is a statutory declaration recording your intention to abandon your old name and adopt a new one.
- Once this is done, the individual must then go to the Office of the Judicial Secretary, where the affidavit is signed/ endorsed.
- The next step applicants need to announce publicly that his/her have made the name change. This can be done by publishing your new name in at the Ghanaian gazette and least one recognised national daily newspaper. It is only after the new name is published in the gazette/ a newspaper that it is considered legally changed. You should ensure that the new name reflects in your vital documents.
- The final step is applying for new government documents reflecting your new acquired name. This can only be done once the new name is published in the Ghana newspapers. A copy of the newspaper has to be attached to the application documents for the new documents.
- The processing time for this procedure takes 2 -8 weeks.

Eligibility
- If you are 16 or 17 year old, you don’t need to have the consent of your parents or anyone with parental responsibility for you to change your name –legally you have the right to choose your own name.
- All British nationals can change their name by applying for a deed poll or by any of the alternative methods described on this site (e.g. by marriage).

Need for the Document
- People choose to change their name for a variety of reasons. Maybe you just want to change the spelling of your name, you don't like the name your parents gave you, you want to get rid of your middle name, you want to remove any family associations, or maybe you just want a change. You can legally change your first name, middle name or surname, or any combination of those.
- How you should go about changing your name will depend upon which category you fit into. Changing your name after marriage or divorce is quite a simple process, while changing your name 'just because' requires a little more effort, but it is still quite straight forward and a lot easier than it used to be.
- To change your name, submit a petition to your local Superior Court that details the reasons you'd like to change your name. After the court approves your request, you'll then need to update your Social Security card, driver's license and passport.
