Apply In-Person:

1. To Apply for Renew Manufacturing and Formulation License in Ghana, the applicant must visit the Food and drugs authority (FDA) regional office in Ghana. Contact link
2. Then, the applicant must obtain the application form from the relevant department.
3. Read carefully through the application form as there contain different section.
4. Please complete each section of this application form as a Word document then submit printed version of the completed form along with the copies of the supporting documents to the FDA regional and district office or the head office.
5. Make sure that his/her have all the documents listed in the “Required Documents” section of this page.
6. A fee for this procedure is has to be paid as per as authority’s request.
7. Submit the completed form in two copies along with the supporting documents to the respective office.
8. This application will be processed and information will be verified by the FDA.
9. If the Expert committee formed by the FDA are satisfied that all the conditions are meet as required for the application, the applicant will be notified of the outcome and issued with the certificate by the register/authorized official.
10. Generally, the processing time for this procedure takes 1 month.

• Identification document (ID card, Passport, Birth certificate )
• A certified true copy of Certificate of Incorporation and Certificate of Commencement of Business
• Copy of initial certificate issued
• Proof of payment of the prescribed fee for the licensing of the facility
• Original expired license
• Any other documents that may be request by the FDA.

• The license given to the manufacturer is valid only for a certain period of time after which it has to be renewed to ensure the quality of formulator manufacturing is maintained.
