How to Apply :
In-Person Application :
- Obtain the application form from any of theEnvironmental Protection Agency (EPA) regional office or head office in Accra, or download it via the EPA website portal Website or click the link below to download(Application form)
- Read through the application form carefully as it contain different section.
- Submit the completed forms in duplicate (2 copies) along with the supporting documents (listed below) at the EPA office for verification and processing.
- You will be issued with the prescribed fee invoice, visit any ECOBANK branch near you with the invoice and make the payment.
- The EPA will dispatch an inspector to the proposed site to conduct an environmental assessment impact study.
- A copy of the report finding will be given to the applicant and if any modification or upgrading is required,Corrective Action and Preventive Action Report (CAPA) report will be issued and you will be required rectify the problem.
- Upon successful implementationof the CAPA, a follow-up inspection may be conducted by the inspection team to ascertain the effectiveness of the implementation (where applicable).
- If the authorized official is satisfied that all the conditions are meet as required for the application, you will be notified of the outcome and issued with the licences, upon payment of all required fees.

- Scan the accurate completed duly filled form along with the supporting documents and send them to the EPA email address ccmc.dept@epa.gov.gh for verification and processing.

- Note :
- All fees charged are payable at any ECO BANK Ghana Branch by presenting the invoice issued by the Agency to the bank.
Required Documents
- Citizens need to be in possession of a valid national identity card copy.
- Companies need to be in possession of a valid company's registration certificate copy.
- Evidences of payment of the prescribe fees(processing and licence fees)
- Any other documents that may be requested by the FDA.
- Note :
- When submitting any supporting documents, please label the documents as indicated in this part (e.g., Attachment A, etc.)
- If the space provided on the application form is not sufficient, additional information required may be provided on a separate document and attached.

DECLARATION BY THE APPLICANT
Need for the Document
- The Environmental Protection Agency Act 1994, Act 490 and Ghana Environmental Assessment Regulations 1999, LI 1652 requires that undertakings likely to have significant impacts on the environment e.g., drilling of Wells, Production of Petroleum, Construction of a Pipeline, must register with the EPA and obtain environmental permits before commencement of construction and operations.
