1. To obtain a Certificate of Life, go to your local Civil Status Office.
2. Do not forget to bring a valid identification. The registry personnel may ask for your ID in order to confirm your identity.
3. Pay the application fee and wait for instructions from the Registrar regarding when can you comeback to collect the certificate.
