How to Apply:

1. You should visit the Prefects and Sub-prefects office near you and express your interest.
2. Fill out the application form, obtained from the above office, attach all the supporting documents listed below and submit them for verification and process.
3. Your biometric information (finger print and photo) will then be taken on site and you will be issued with an acknowledgement slip.
4. You should carry the acknowledgment slip when coming to pick the ID card.

• Present a legalized and certified birth certificate conforming to the original
• Three (3) color ID photos
• Copies of Parents identification documents
• Copies of Death certificate of parents if deceased
