How to Apply:

1. You should visit the Caisse Nationale de Sécurité Sociale (CNSS) office near you and express your interest at the service desk.
2. Application form can be obtain at any CNSS branch office or download it via the CNSS website portal or click the link below to download : (occupation accident declaration application form) OR (declaration of occupation disease application form)
3. Submit the completed duly filed application form along with the supporting documents (listed under “Required Documents” section) for verification and processing to the attending official at the CNSS.
4. If the authorized official is satisfied that all the documents are in order you will be notified of the outcome and be given further instruction

• Completed declaration of work accident or occupational disease
• Certificate of first observation or initial certificate or medical certificate
• Certificate of 1st extension if necessary
• Healing or consolidation certificate
• Medical expertise
• Identification document (ID card/valid passport)
