1. To apply for a Unique Number Plate, you will need to go to the office of Land Transport Authority (addresses provided below).
2. You will be asked to complete an application form. A link is provided under 'Documents to Use' section which you can use to download the form.
3. Submit the completed form together with all other required documents (see list below).
4. The Customer Service Officer will check your application, verify and register your application.
5. Application for unique number plate will brought to the Team Leader for review.
6. After the Team Leader has reviewed and verified your application, your application will be forwarded to the Regional Manager for final approval.
7. Once the RM approves your application for unique number plate, you will then be given an approval letter from LTA stating that your application for unique number plate is approved.
8. You will need to pay the Unique number plate.
9. You will need to go to your respective third party policy insurer and have your TPP amended.
10. The Unique number plate will be ordered for printing by the CSO.
11. Once you got your TPP amended go back to the CSO, together with your amended TPP, running number plate, and the current registration label.
12. LTA will update the system and the unique number plate will be issued.
